Captions disappearing

I've gone through and added names & captions for each of my photos so the normal filename doesn't appear on the site. Every once in a while they get reset and lost. Without me even loging in as admin for days.

Does this happen to anyone else? Is it a problem with the automatic scanning of the directories?
